"On 5th October 1944 near Alphen / Holland / MR 988233 / in heavy fighting, when the advance of the Regiment was stopped by the concentrated enemy anti-tank fire he advanced with his tank and took up position and destroyed the enemy Self Propelled gun by thrusting forward under heavy anti-tank fire. His great personal courage and excellent leadership enabled "C" Squadron to advance and to affect the capture of three enemy 105 mm field guns.

Finally his tank was hit by enemy anti-tank fire and Lieutenant Błędowski's legs were shot away".
