"On 2nd October 1944 at Weelde/Belgium MR 095158/ as a Squadron Commander during the attack on the Railways Station, when the advance of the Squadron and a Company of MOT Battalion was held up heavy enemy Anti-Tank, Mortar and Machine gun fire. He advanced alone in his tank in the face of enemy anti-tank fire destroying from an entirely open position an anti tank gun. His action inspired all the soldiers of the Squadron, who without further orders advanced straight into the enemy fire barrage. Due to the conspicuous bravery of Captain Jakubiec and the example set by him the Station was taken in a very short time, with small losses despite heavy enemy opposition."
