"For gallantry in action while serving with Company G, 3d Battalion, 504th Parachute Infantry Regiment, 82d Airborne Division, in action on 27 September 1944, near ****, Holland. Private First Class Jasak was a member of a party taking needed supplies to a surrounded platoon outpost when an enemy machine gun opened fire on them forcing them to seek cover. Private First Class Jasak, single-handedly attacked the machine gun nest, armed only with a Gammon Grenade and a pistol, knocked out the enemy gun and killed two and wounded another member of the crew. His courageous and daring attack enabled the party to deliver the badly needed supplies without loss and reflects the highest credit upon himself and the Armed Forces."
